Sample Submission Agreement

This Sample Submission Agreement ("Agreement") applies to every sample you ("Client") send to SteriGenix Analytical, LLC ("SteriGenix") for analytical testing. It supplements the SteriGenix Terms of Service.
1. Submission packet
Each sample shipment must include:
- A completed sample submission form (one per batch).
- The submission number issued by SteriGenix in advance.
- A shipping manifest identifying contents, mass, and lot.
Samples received without a current submission number may be refused, returned, or held at Client's expense.
2. Packaging and shipping
- Sample vials must be sealed, labeled with batch number, and shipped in tamper-evident secondary packaging.
- Shipments must comply with all applicable carrier and jurisdictional regulations, including any controlled-substance, dangerous-goods, or customs requirements.
- Risk of loss in transit remains with Client until the shipment is logged into the SteriGenix sample-receiving system.
3. Acceptance criteria
SteriGenix may reject any sample that arrives leaking, contaminated, unidentifiable, of insufficient quantity, or accompanied by incomplete paperwork. Rejected samples are documented and either returned at Client's cost or destroyed in accordance with internal SOPs.
4. Chain of custody
From receipt through analysis, samples are tracked under documented chain-of-custody procedures. The chain-of-custody record is retained with the COA for a minimum of three (3) years.
5. Retention and disposition
- Unless otherwise agreed in writing, SteriGenix retains a representative portion of each tested batch for the period stated on the COA (default three months) and may dispose of remaining material thereafter.
- Return shipment of retain material is available on written request and at Client's expense, subject to applicable shipping restrictions.
6. Reporting
Results are issued as a Certificate of Analysis identifying the tested batch by its batch number. Once published, the COA is publicly verifiable at the URL printed on its face.
7. Restricted materials
SteriGenix does not accept materials that the Client is not lawfully authorized to possess and ship. Submission of such materials is a material breach of this Agreement and grounds for refusal of all future service.
8. No clinical use
Submitted materials are accepted only on the express representation that they are research-grade and not intended for human or veterinary use. Client agrees not to misrepresent any SteriGenix-issued COA to suggest otherwise.
9. Acceptance of this Agreement
Submission of a sample to SteriGenix constitutes acceptance of this Agreement and incorporates the SteriGenix Terms of Service and Privacy Policy in full.
